Community Health Center is pleased to offer school-based health services in your child's school during the school day.

sick childWhen to Keep Your Child Home In order to protect the health of all students and staff, we ask parents/guardians to keep their children home when they are ill. The following link will provide information on when children must stay home from school and when the school nurse will call parent/guardian if a student becomes ill at school and needs to be picked up. Immunization Requirements

Please consult your child's pediatrician to be sure your child has received all required immunizations. Immunization Regulations

Physical Exams

Students entering Kindergarten, Grade 7 and Grade 11 are required to have a Physical Exam. The exam must be completed and submitted on the blue State of Connecticut Health Assessment Record form.
